Application
31-28-2-1 Sec. 1. This chapter applies to children who receive
foster care that is funded by the department or a county office.

Medical treatment records; forms
31-28-2-2 Sec. 2. (a) If medical care is provided to a child who
receives foster care, the person who has custody of the child shall
inform the provider that the provider is required to file a copy of:
(1) the form provided under IC 31-28-3; and
(2) the child's medical treatment record for the medical care;
with the county office in which the child resides.
(b) The provider shall file the form and record with the county
office.

Maintenance of records
31-28-2-3 Sec. 3. The county office shall maintain the medical
treatment records filed under section 2 of this chapter.

IC 31-28-2-4
Copies to foster parents
31-28-2-4 Sec. 4. The county office shall provide a copy of the
medical treatment records filed under section 2 of this chapter to the
person who provides foster care to a child.
