                               AIR CARRIER ACT
    61-1105.  APPLICATION -- FORM AND PROOF. An applicant shall submit his
written verified application to the commission, and a duplicate application to
the director. The application shall be in such form and contain such
information and be accompanied by proof of service upon all air carriers with
which the proposed service is likely to compete and such other interested
parties as the commission requires.
