Petitioner, a state prisoner proceeding pro se, has filed an application for a writ of
16

habeas corpus pursuant to 28 U.S.C. § 2254. Petitioner has not, however, filed a completed in
18
forma pauperis affidavit or paid the required filing fee ($5.00). See 28 U.S.C. §§ 1914(a);
19
1915(a). Petitioner did submit an application for in forma pauperis status, which he signed, but
20
he did not fill it out with any of the information requested on the form. He has also filed a
21
motion for an extension of time in which to obtain a certified statement of his trust account funds
22
from the prison trust office. Insofar as this order gives petitioner additional time from this date in
23
which to complete the application process, that motion is moot. Petitioner need only submit the
24
enclosed application, providing all available information and signatures requested therein, in the
25
next thirty days. Petitioner may also submit the $5.00 filing fee instead of the application.

In accordance with the above, IT IS HEREBY ORDERED that:

1. Petitioner shall submit, within thirty days from the date of this order, an
3
affidavit in support of his request to proceed in forma pauperis or the appropriate filing fee;
4
petitioner’s failure to comply with this order will result in the dismissal of this action.
2. The Clerk of the Court is directed to send petitioner a copy of the in forma
5
6
pauperis form used by this district.
3. Petitioner’s motion for additional time in which to submit a prison trust
7
8
account statement (Docket No. 9) is moot.
